WHY CHOOSE BLUE MOUNTAIN WELLNESS?

Our practice is committed to delivering a level of personalized care that is professional, discreet, and confidential. We will always provide a respectful, nonjudgmental, and collaborative experience. We are readily accessible to all patients so that we can partner with them to ensure the best possible clinical outcomes.

WHAT CAN I EXPECT ON MY FIRST VISIT?

You will meet with a medical provider who will ask you questions about your medical and mental health history. All consultations are confidential. Based upon the medical assessment, the provider will work with you to develop a treatment plan and, if indicated, prescribe medications.

WHAT FORMS OF PAYMENT ARE ACCEPTED FOR SERVICES?

Payment is accepted in the form of Cash and Debit/Credit Cards. We do not accept personal checks.

DO YOU ACCEPT INSURANCE?

We are an out-of-network provider. We do not accept direct payments from any insurance company. We will assist our patients who are seeking reimbursement from an insurance company. Please contact your insurance company directly to obtain information about your “out-of-network” coverage for outpatient mental health services. Our office will provide detailed invoices by request to submit claims to insurance companies for reimbursement purposes. Services are an eligible expense for Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA).

WHAT IS THE CANCELLATION POLICY?

We understand that everyone has unexpected changes to their schedule. With advance notice, we will try to accommodate any schedule change requests. However, without 48 hours advance notice, you will be charged for missed/canceled sessions. We charge a $20.00 no-show or $20.00 cancellation fee if canceled within 12 hours of scheduled appointment time.
